چکیده

Sin,—Hyperglycinsemia, hyperglycinuria,l propionicaciduria2 and Reye’s syndrome3 have been reported in epileptic patients taking the anticonvulsant drug valproic acid (VPA). While investigating possible mechanisms of VPA toxicity we found increased plasma glycine and propionic acid concentrations in symptom-free patients taking VPA (table i). With informed consent, loading tests with 500 mg VPA orally were done in three children who had experienced severe vomiting, lethargy, or coma while on VPA. Plasma propionate rose from baseline 5 to 82 nmol/1 (case 1), 21 to 81 nmol/1 (case 2), and 14 to 62 nmol/1 (case 3) by 3 h after the dose. However, these data did not explain why some children became ill from VPA; these levels of glycine and propionate are not nearly as high

Valproate-induced hyperammonemic encephalopathy with normal liver function.

Hyperammonemic encephalopathy with normal liver function is an uncommon serious adverse effect of valproate therapy. We retrospectively analyzed the case records of 5 patients of epilepsy on valproate with hyperammonemic encephalopathy. Of the 5 patients, 3 were on monotherapy. The mean valproate dose was 1250 mg/day and the duration of therapy ranged between 4 and 90 days. Alteration in the se...

Hyperammonaemic encephalopathy after initiation of valproate therapy in unrecognised ornithine transcarbamylase deficiency.

Ornithine transcarbamylase deficiency is an X linked disorder and the most common inherited cause of hyperammonaemia. Fluctuating concentrations of ammonia, glutamine, and other excitotoxic amino acids result in a chronic or episodically recurring encephalopathy. A heterozygous female patient first presented with protein intolerance, attacks of vomiting, and signs of mental retardation in early...

Valproate-induced encephalopathy: assessment with MR imaging and 1H MR spectroscopy.

The anticonvulsant agent valproate (VPA) may cause hyperammonemic encephalopathy. Magnetic resonance imaging (MRI) and proton MR spectroscopic (MRS) findings in a patient with VPA-induced hyperammonemic encephalopathy are described. MRI showed a metabolic-toxic lesion pattern with bilateral T2-hyperintense lesions in the cerebellar white matter and in the globus pallidus. MR spectroscopic findi...

Non-hyperammonaemic valproate encephalopathy after 20 years of treatment

Sodium valproate is a commonly used antiseizure drug with broad indications for different seizuretypes and epilepsy syndromes. Well-recognised side effects include weight gain, tremor, dizziness, and unsteadiness. Non-hyperammonaemic parkinsonism, with or without cognitive impairment, is a rare adverse effect of sodium valproate. We present the case of a sixty year-old lady with a generalized s...

Pronounced reversible hyperammonemic encephalopathy associated with combined valproate–topiramate therapy in a 7-year-old girl

Valproate is one of the most frequently used anticonvulsive drugs in children and adults. Valproate is a generally well tolerated medication. However, encephalopathy with or without hyperammonemia is one of its rare adverse events. We present a 7-year-old girl who suffered from epilepsy with generalized tonic-clonic seizures and absence epilepsy. She was initially treated with topiramate. Methy...
